This is a story of a 22 years old patient who was born to less educated parents in remote area and reared as Female in early life. She passed all her exams in both her school and graduation level. Not a single instance has come in her life when she ever felt ambiguity in her sexual behaviour. As a normal human being, she continued her studies and went to Mumbai. She applied for a job in CISF as a female candidate and luckily got selected for the job. After selection, she started her training in the training college.

During training, abnormality of genital was noted by examining doctors, who referred her case to detailed examination to bigger hospital. During her examination, detailed ultrasound revealed absence of uterus. She was referred to AIIMS, New Delhi for expert opinion. After expert opinion, she was asked to leave the training as her selection was cancelled on the grounds that she applied as female while her generic sex was 46 XY, which was consistent with males.

During her medical examination, abnormality of genitals was noted by examining doctor and detailed examination was advised. During this examination ultrasound of abdomen revealed absence of uterus. She was referred to AIIMS in Delhi for expert opinion. After expert opinion, she was asked to leave the training as her selection was cancelled on the ground that she applied as 'female' while her genetic sex was 46XY (consistent with male). At this stage, she contacted us for help through a senior doctor.

Medical status: At AIIMS, she was diagnosed as having a rare hormonal disease. There are two conditions:-

1 5 alpha reductase deficiency: This is a rare disease where male sex hormone is not converted to another male hormone (testosterone (Te) in not converted to Dihydrotestosterone (DHT), this conversion is by enzyme '5 alpha reductase' and in absence of this enzyme, DHT is not formed in the body. DHT is an essential hormone especially for exact formation of external genitals of male. If DHT is not there, external genitals of male looks like a female. Severity of abnormality depends upon severity of deficiency, in severe cases, genitals may be like just female) reversal of sex is known phenomenon in this rare disease and the most common reversal is change of sex from female to male. This rare condition is not common in India but we see few cases in a year. Treatment of this condition is surgery. DHT creams are not available in India.


2. Partial androgen resistance: this disease is again very rare in general population and occurs as one in few million people. In this condition, male hormone (Te) is not fully effective and there is partial resistance to the actions of this hormone. As a result, there is problem in formation of external as well as internal genital organs. Severity of this condition depends upon degree of resistance to actions, if resistance is complete, Pt will present as complete ‘female’ with external genitals of female but no uterus and ovary but both testes are present in abdomen and genetic structure is 46 XY (male). Complete resistance is also known as Testicular feminization syndrome or complete androgen resistance. In partial resistance, genitals are in between male and female. Sex reversal is again known phenomenon I this condition. Treatment of this condition is surgery. Sex of rearing depends upon ambiguity of genitals and wish of patient.It is very difficult to differentiate between these two conditions as special genetic tests are required.

Conclusion:

Now this Girl wishs to lead a normal life of a boy, Doctors are in full support of her and according to the team of treating doctors, CISF should not discriminate her on the basis of her current gender, other important issue is that this girl gave her 12th board exam a female so she was forced to do further studies as a female. Considering her background CISF should take her back to the service and also she should be transferred to CISF in Delhi so her further medical needs can be take care.

To espouse her case, few of the points even raised by me being among the member of a social organization called caremycolostomy.org, the same read as under:

The fundamental questions remain the same despite the enormous progress we have made as human beings. How can we fight inequality? How do we ensure a stronger life for the weaker? How do we create a hopeful life for the desperate? The Human race has overcome so many challenges over the years that nothing seems impossible! However, we at “Care My Colostomy” personally believe that we have not even achieved one tenth of what is possible when it comes to creating socially acceptable living condition for all. Alleviating current social burden to unfold the full potential of communities should be the guiding spirit of all of us as humans. Lets take the present issue of this poor girl who despite having made no offence being victimized from all corners.


Gender differences is a distinction of biological and / or physiological characteristic typically associated with either males or females of a species in general. In the study of humans, social – political issues arises in classifying whether a sex difference result from the biology of gender. For example, men are taller then women on average, but an individual woman may be taller then an individual man. Similarly, women give birth to babies; men never do. Gender archaeology itself is based on the ideas that even though nearly all individual are naturally born either male or female that there is nothing natural about gender. It is believed that gender is actually a social construct which varies between cultures and changes through times.


The existence of a gender difference does not necessarily identity whether the trait is due to nature or environment. Some traits are obviously innate (for example, reproductive organs), others obviously environmental (for example, given names), while for others the relationship is either multi causes or unknown.


Sex, race and sexual orientation are all characteristic that – in almost all instances – have absolutely no bearing on societal functioning (and are in no way chosen by the one holding the characteristic). The fact that the job applicant is a women, or of particular national origin, or is a gay, have no bearing on the performance of a job at hand. While there are admittedly some instance where such a factor might be relevant (surrogate motherhood, for example), those are extraordinary few – and must be shown by actual evidence to be relevant. So there is a crystal clear line as to when we can choose to deny rights to citizens. Citizens who have (a) fulfilled the conditions for full citizenships, and (b) have done nothing criminal to justify the removal of any of those rights, are entitled to all the rights guaranteed to any other citizens. Furthermore, the use is restricted to contexts in which societal, rights are in a position to be denied or restricted (jobs, government, education etc). As a participants in a society, we are welcome as individual to associate with whom we like so long as that association is completely voluntary and unconnected to one of the aforementioned contexts. In our proposed example, however, the idea of “discriminating” against someone who has committed a crime is a complete red herring – it has absolutely no connection whatsoever to the question at hand (and, in fact is improper use of the word “discriminate”). The removal of an individual’s right (for example, by incarceration) is based on actual, relevant information – the determination of criminal offence by the court of law. The decision making factors are directly related to the decisions directly made. But in the present case no felony has been committed by the applicant still she is being subjected to all mental and physical suffering.

To lesson the grievance of the female herein, we at caremycolostomy.org has initiated a crusade, wherein a helping hands have been invited from all corners of the world. To espouse her cause one of our founder member namely Mr. Rahul Verma has even uploaded one petition for the information of the public at large for support of this noble cause.
